Vaccine Information Statements
Vaccine Information Statements (VIS) are designed to explain the benefits and and possible risks of a vaccine to recipients of a vaccine, their parents, and/or their legal representatives. It is a Federal law that Vaccine Information Statements be handed out whenever certain vaccinations are
administered. The Centers for Disease Control and Prevention (CDC) has created a flyer for physicians and providers called CDC Instructions for the Use of VIS .
How to find the revision dates on Vaccine Information Statements – The revision date is located on the back of each Vaccine Information Statements towards the bottom. Be sure your clinic or office is using only the most current Vaccine Information Statements. When you receive an updated version, please discard any outdated VIS immediately. It is important to distribute only the most up-to-date VIS.
